Submission Requirements
- A Complete nomination form.
- A 300-word maximum, typed summary outlining specific examples that depict the nominee's empathy, support and respect for cancer survivors and their loved ones, how the nominee encourages leadership and participation in decision-making by loved ones or how the nominee is attentive and compassionate to the needs of survivors and their loved ones. Include a brief statement on why you feel they deserve the award. If you are nominating a business, you may wish to include how the business shows support, leadership and compassion for survivors and their loved ones. Please note that this summary will be presented to your nominee and may be used for publication purposes.
- We will invite nominees to attend the event at a reduced cost of $20.
- You may nominate more than one person, but please use only one form per nominee.
The nomination deadline for the 2008 People's Caring Awards is April 30, 2008.
